Position Overview

The Community Health Representative (CHR) plays a vital role within the LFN Health Team, focusing on the delivery of community health programs aimed at prevention, intervention, and treatment. This position requires a proactive individual who can effectively communicate with patients, families, and healthcare providers to ensure comprehensive care and understanding of health conditions.

The CHR will also monitor community health trends, especially during epidemics and emergencies, and will contribute to developing educational activities to promote health awareness. This role is crucial in fostering a healthier community through dedication and informed health practices.

Ideal Candidate Profile

We are seeking a passionate and self-motivated individual with a strong desire to improve community health outcomes. The ideal candidate will possess the following qualifications:

  • High School Diploma or equivalent.
  • Valid First Aid (Level 1) and CPR Certificates.
  • A minimum of 1 year of experience in community health program delivery, preferably in a First Nations context.
  • Current criminal record check.
  • Valid BC driver’s license, driving abstract, and access to a personal vehicle.

The successful candidate will demonstrate a commitment to community engagement, effective communication skills, and the ability to work collaboratively within a team. A strong understanding of health promotion and the needs of First Nations communities will be an advantage.
